Billing for psychotherapy sessions via telehealth during COVID 19 crisis

Many questions have come up about how insurance companies want telehealth sessions billed.  Telehealth sessions are indicated with a modifier along side the regular psychotherapy CPT codes in box 24D of the CMS 1500 form.

All telehealth services are billed as place of service "02" in box 24B of the CMS 1500 form.  Medicare and Medicaid Connecticut do not require a modifier when billing telehealth services but you do have to bill with 02 place of service.  BUT Medicare will pay more if you bill it as POS 11 and 95 modifier.

The two modifiers you must use to indicate telehealth sessions were performed are:

GT: Via interactive audio and video telecommunication systems
95:  Synchronous telemedicine service rendered via a real-time interactive audio and video          telecommunications system

Below is a list of some popular companies and which modifier they prefer you use.

GT - Beacon, Quest, United, Aetna, BCBS NY, BCBS NJ, BCBS ILL, Highmark BCBS, Independence BCBS, Carefirst BCBS, GHI

95 - Cigna, BCBS TX, Healthfirst, Fidelis, BCBS VT.
